There are a number of separate charges associated with your procedure at Fremont Surgery Center. Your bill from Fremont Surgery Center covers use of our facility and equipment, supplies, nursing and support staff, all medications, and fluoroscopy/xray services.  You may receive charges from several other providers in addition to your facility bill from Fremont Surgery Center:

• Anesthesiologist

• Surgeon

• Pathologist

• Durable Medical Equipment provider (for crutches, walker, sling, boot, sequential compression sleeves, etc.)

 

Full payment is due within 30 days from your date of service.  You are responsible for guaranteeing payment on your account and being aware of your individual insurance policy restrictions and benefits.  Our relationship is with you as our patient, not your insurance company.  Consequently, all charges are your responsibility.

 

Fremont Surgery Center’s policy is to turn all delinquent accounts over to a collection agency.

Please note that you will need your account number (from your statement) when paying your bill.  Enter this number in the “invoice number” section of the form.
