Billing and Insurance
Prior to your procedure a representative from Fremont Surgery Center will contact you by phone to answer your questions about your bill and insurance. Our representative will also discuss payment options, including your insurance coverage, co-payments, deductibles, and any cash payment that will be due at the time of service.
We accept payment from all insurance plans, including workers compensation and Medicare. We are contracted providers for many private insurance plans. As a courtesy, we will help with any pre-authorization issues. We will also file your insurance claim for you if you sign an "assignment of benefits form." That form directs your insurance company to send payment directly to Fremont Surgery Center.
Fremont Surgery Center requires that you pay any amount not covered by your insurance company--such as deductibles, co-payments and co-insurance--on the day of your procedure unless you make prior arrangements with us. For your convenience we accept cash, personal checks, cashier's checks, money orders, and payments through all major credit cards, including Visa, MasterCard, Discover and American Express.
Please note that our bill does not include any services from your doctor, an anesthesiologist or pathologist, or a laboratory. You will receive separate bills from them. You may also receive separate bills from Fremont Surgery Center and your insurance company.
